Forms

Forms can not be completed or signed until all sections that must be completed by the parent or guardian of the child are completed. When dropping off and picking up forms; please do so during regular office hours and have a form of identification.

Physical Exams:

There is no charge for forms completed on the day of the exam or within 30 days of the exam. Otherwise, the fee is $10.00

For other forms such as FMLA the fee is between $5.00-$15.00

When dropping off a form please allow 48 hours for completion.
