Missed Appointments for Initial Visit:
If an Initial Visit appointment must be cancelled or rescheduled we request a call 48 hours in advance. Missed appointments are disruptive to our schedule as well as depriving other patients of an appointment opportunity. We realize there may be extenuating circumstances, and if you notify us we will take this into consideration.
First Visit: An office charge will be made commensurate with the time allowed for your child's appointment. This charge must be paid within 10 days, in order to become a patient in our practice. You will then be able to reschedule your child's initial visit.
Second Visit: Another charge will be made and your child will not be eligible to become a patient in our practice.
Missed Appointments for established patients:
If an appointtment must be cancelled or rescheduled we request a call 24 hours in advance. Missed appointments are disruptive to our schedule as well as depriving other patients of an appointment opportunity. We realize there may be extenuating circumstances, and if you notify us we take this into consideration.
First Visit: Will be excused
Second Visit: An office charge will be made commensurate with the time allowed for you child's appointment. The charge must be paid within 14 business days.
Third Visit: You will be asked to select another physician for pediatric care. A 30 day grace period will be allowed for emergency care.
